Restaurantinspections for Nov. 2

The following restaurants were inspected by the Lake Cumberland District Health Department using the scale below.

- 85 to 100, pass inspection.

- 70 to 84, restaurant must be reinspected within 30 days.

- Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the restaurant will be closed.

- Benningfield's Country Market of Old Lebanon Road in Campbellsville received a food health code score of 91 and a retail health code score of 96, citing seven violations. Raw/uncooked hamburger was stored next to or above ready-to-eat foods in a kitchen refrigerator, no hair restraints were worn during food preparation, wiping cloths were found at mop/hand sink not in a sanitizing solution, the inside bottom of bread trays wasn't clean, kitchen utensils were stored incorrectly with the handle in, there was no soap or hand towels at a mop/hand sink in the kitchen, the men's restroom door was in poor repair and wooden floors throughout the retail/kitchen area were in poor condition.

- The Magic Wok of Campbellsville Bypass in Campbellsville received a health code score of 88, citing nine violations. Unlabeled bulk containers of dry goods were found in the kitchen storage room, there were no signs at the buffet instructing patrons to get a clean plate at each visit, potentially hazardous foods, raw meats and poultry, were stored above or next to ready-to-eat foods in a walk-in cooler, uncovered foods were found in a walk-in cooler, plastic bowls without handles were used as scoops for bulk dry goods, the inside of a Pepsi reach-in cooler in the kitchen wasn't clean, the exterior of food storage containers in walk-in cooler and dry storage good containers weren't clean, floors throughout the kitchen and in walk-in cooler/freezer were very unclean in hard-to-reach areas such as under and behind shelving and equipment, the walls of the kitchen were soiled, unshielded lights were found over the hot and cold buffet bar and the men's restroom was in poor repair.

- Adanta of Old Lebanon Road in Campbellsville received a health code score of 100, citing no violations.

- Taylor Regional Hospital of Old Lebanon Road in Campbellsville received a health code score of 100, citing no violations.

- Walmart #665 of Campbellsville Bypass in Campbellsville received a food health code score of 98 and a retail health code score of 99, citing three violations. Floors in walk-in coolers weren't clean in hard-to-reach areas, metal holders for baking trays weren't clean and light shields in a walk-in cooler were in poor repair.

- Campbellsville Child Development Center of North Central Avenue in Campbellsville received a health code score of 100, citing no violations.
