Restaurant inspections for July 27

The following restaurants were inspected by the Lake Cumberland District Health Department using the scale below.

- 85 to 100, pass inspection

- 70 to 84, restaurant must be reinspected within 30 days

- Under 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the restaurant will be closed.

- Montgomery Summer Snow of Old Lebanon Road in Campbellsville had a grand opening inspection, citing three violations. Single service items were stored on the floor, a handwashing station was improper and lighting fixtures weren't shielded. Health code scores are not given for opening inspections.

- T-Mart of East Broadway in Campbellsville received a health code score of 94, citing five violations. A cabinet under a soda machine wasn't clean, single service items were stored on the floor, there was no hot water or soap in the women's restroom and ceiling tiles were in poor repair throughout the store.

- Gate Service Station #620 of East Broadway in Campbellsville received a food health code score of 80 and a retail health code score of 94, citing 11 violations. Weather stripping was worn at the back door, there was no mop sink, no backflow preventor on hose hookup, no thermometers were found in a refrigerator or freezer, no test strips were near the three-compartment sink, there was buildup in an icebox, single service items were stored on the floor, there was no lid on the trash can in the women's restroom, shelving wasn't smooth and easily cleanable and no thermometers were found in a walk-in unit. At a follow-up inspection, the station received a food health code score of 98 and a retail health code score of 100, citing one violation. There was no lid on a trashcan in the women's restroom.

- Orville's of South Central Avenue in Campbellsville received a health code score of 80, citing 12 violations. A door's seal was worn, a restroom door wasn't self closing, a trash can wasn't covered in the women's restroom, lights weren't shielded, walls were in poor repair, lids were stored in an unclean container, floors weren't clean in hard-to-reach areas, rugs near a grill weren't easily cleanable, shelving wasn't smooth or easily cleanable, a freezer lid was in poor repair, the blade on a can opener was in poor repair, duct tape was found on a food preparation counter, a bungee cord was found in the food preparation area, beans were stored in an unapproved container, potatoes were stored uncovered in a cooler, ground beef was stored next to cabbage in a cooler, shelving was heavily soiled in a cooler, a grill head was soiled and a milkshake machine wasn't clean. At a follow-up inspection, the restaurant received a health code score of 94, citing five violations. Lights weren't shielded, walls were in poor repair, floors weren't clean in hard-to-reach areas, rugs near a grill weren't easily cleanable, the lid of a deep freeze was in poor repair and a meat slicer was soiled.

- Indian Ridge Campground of Campbellsville received a health code score of 100, citing no violations.

- Domino's Pizza of East Broadway in Campbellsville received a health code score of 100, citing no violations.

- Five Star Food Mart of East Broadway in Campbellsville received a food health code score of 95 and a retail health code score of 97, citing five violations. Eggs were stored above or next to ready-to-eat foods in a kitchen preparation reach-in cooler, wiping cloths were found in an island cabinet drink station, not in sanitized solution, counters at drink machines and cabinets under coffee makers and drink machines weren't clean, lights in the kitchen area weren't shielded above the oven and preparation table areas, and mops were stored with the head down at a mop sink area.
