Restaurant inspections for Dec. 28

The following restaurants were inspected by the Lake Cumberland District Health Department using the scale below.

- 85 to 100, pass inspection

- 70 to 84, restaurant must be re-inspected within 30 days

- Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the restaurant will be closed.

- Creek Side Restaurant of New Lebanon Road received a health code score of 86, citing nine violations. In a reach-in refrigerator, there was no lid on butter, no lid on a tea batch, no lid on seasoned salt and cornstarch, rolls weren't covered, potentially hazardous foods were stored over ready-to-eat foods, food was stored on the floor of a walk-in freezer and walk-in cooler, a brush stored in butter was not food grade, no thermometers were found in a refrigerator/freezer, walls were heavily soiled throughout and the ceiling was in poor repair, shelving and a hood were heavily soiled, ice and sugar scoops weren't food grade and were improperly stored, floors were heavily soiled in hard-to-reach areas, heat lamps weren't properly shielded and a dishwasher wasn't reaching 180 degrees for sanitizing. At a follow-up inspection, the restaurant received a health code score of 96, citing three violations. Floors weren't clean in hard-to-reach areas, walls weren't clean and the ceiling was in poor repair and a dishwasher wasn't reaching 180 degrees.

- Minit Mart #52 of Old Lebanon Road in Campbellsville received a food health code score of 96 and a retail health code score of 97, citing four violations. A potato chip display was stored directly on the ground, an ice cream reach-in chest cooler was in need of defrosting, a meat slicer and the inside of an ice machine weren't clean and a ceiling tile in the retail section was water damaged.

- Pizza Hut of Campbellsville Bypass in Campbellsville received a health code score of 89, citing eight violations. A dishwashing temperature gauge wasn't working at a mechanical dishwasher, the inside of a microwave wasn't clean, pizza pans had carbon build-up, preparation area tables had grease buildup, the exterior top of a wing warmer was soiled, cabinets under a fountain drink machine weren't clean inside, countertops under a counter at a drink area weren't clean, preparation bowls and storage shelving weren't clean, no soap was found at a hand washing station next to a dishwasher, no hand towels were found at a hand sink in the preparation area, floors throughout the kitchen were soiled, a wall in a section had grease build-up, ceiling tiles throughout the kitchen were soiled and lighting in a section was inadequate.

- Subway #6815 of Campbellsville Bypass in Campbellsville received a health code score of 94, citing five violations. No conspicuous thermometers were found in a dining area reach-in cooler, sandwich preparation table cutting boards were in poor repair and were pitted and not smooth for proper sanitizing, the inside of an ice machine was rusty, shelving in a storage area wasn't clean, water damaged tiles were found in a storage/preparation area and lights in a preparation/dishwashing area were unshielded.

- Learning Tree Child Center of Campbellsville Bypass in Campbellsville received a health code score of 100, citing no violations.

- God's Rainbow Inc. of Bennett Lane in Campbellsville received a health code score of 99, citing one violation. No chemical test kits were found.

- Windsor Gardens of Garden Way in Campbellsville received a health code score of 100, citing no violations.

- Smoke Pit BBQ of New Columbia Road in Campbellsville received a health code score of 100, citing no violations.

- Tim Horton Camp Kentahten of Kentahten Trail in Campbellsville received a health score of 98, citing one violation. Frozen goods were stored on the floor of a walk-in freezer.

- Winters Dining Hall of University Drive in Campbellsville received a health code score of 96, citing three violations. Food and buns were stored on the floor of a walk-in freezer, sugar/flour bins and shelving weren't clean, and floors weren't clean in hard-to-reach areas.

- WHI Hardwood Café of University Drive in Campbellsville received a health code score of 100, citing no violations.

- Gate #620 of East Broadway in Campbellsville received a food and retail health code score of 99, citing one violation. Shelving in a walk-in cooler wasn't smooth and easily cleanable.

- Jeff's Food Mart of Meader Street in Campbellsville received a health code score of 99, citing one violation. There was no hot water in the restroom.
