Restaurant inspections for April 13

The following restaurants were inspected by the Lake Cumberland District Health using the scale below.

- 85 to 100, pass inspection.

- 70 to 84, restaurant must be reinspected within 30 days.

- Under 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the restaurant will be closed.

- Pizza Hut of Campbellsville Bypass in Campbellsville received a health code score of 93 citing five violations. A dishwasher temperature gauge was in poor repair, pizza pans had char build up, a reach-in cooler wasn't clean, the exterior of an ice machine wasn't clean, a dishwasher/dishwasher area wasn't clean, ceiling tiles weren't clean and less than required lighting was found in a food preparation area.

- Panchos of East Broadway in Campbellsville received a health code score of 95 citing four violations. Tea wasn't covered, shrimp in freezer wasn't covered, fried ice cream in freezer wasn't covered, cheese sauce wasn't maintaining its proper temperature, sugar packets and paprika were stored in utility closet, raw meat was stored above cheese in a refrigerator, wiping cloths weren't stored in a sanitizing solution, forks were stored with their handles down and there was no lid on a trashcan in women's restroom.

- The Shack, no address given, received a health code score of 100 citing no violations.

- Fiesta Mexico of East Broadway in Campbellsville received a health code score of 100 citing no violations.

- Hawg Daddy's, no address given, received a health code score of 96 citing four violations. The ceiling had a water leak, floors weren't clean in a storage area, lighting wasn't shielded and surfaces weren't smooth and easily cleanable.

- Cornerstone Convenient Store of New Columbia Road in Campbellsville received follow up food and retail health code scores of 97 citing three violations. An ice machine drip pan lid was rusty, canned goods and shelving were dusty, a cabinet below a fountain drink machine wasn't clean and lights on a hot bar weren't shielded.

- Creek Side Restaurant of New Lebanon Road in Campbellsville received a follow up health code score of 98 citing two violations. A floor in the dishwashing area was in poor repair and had paint chipping and a wall and ceiling tiles at a dishwashing area were water damaged.
