Restaurant inspections

The following restaurants were inspected by the Lake Cumberland District Health using the scale below.

- 85 to 100, pass inspection

- 70 to 84, restaurant must be re-inspected within 30 days

- Under 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the restaurant will be closed.

- Windsor Gardens of Campbellsville received a health code score of 97, citing two violations. Rinse water at a chemical sanitizing dishwasher was not reaching its proper temperature and clean forks and spoons were stored improperly.

- Taylor County Elementary School of Lebanon Avenue in Campbellsville received a health code score of 100, citing no violations.

- Burger King of Campbellsville Bypass in Campbellsville received a health code score of 100, citing no violations.

- Campbellsville Middle School of Roberts Road in Campbellsville received a health code score of 97, citing three violations. Floor tiles in kitchen were in poor repair, the ceiling in a storage area was in poor repair, paint on pipes in the kitchen was chipping and a light shield over an oven was missing.

- Family Dollar #651 of Central Shopping Center in Campbellsville received a health code score of 100, citing no violations.

- Houchens #33 of Campbellsville received a health code score of 96, citing three violations. A back door storage room/produce area door sweep was in poor repair, floors were in poor repair in the meat processing area, lights weren't shielded in meat/ice cream cooler and a petition was needed between raw meat and cheese.

- Cornerstone Convenience Store of New Columbia Road in Campbellsville received a food health code score of 89 and a retail health code score of 92, citing eight violations. Mice/rodent droppings were found in a cabinet below a fountain drink area in retail section and below a meat slicer in the kitchen and proof/documentation of abatement program will be required, bagged ice was improperly labeled, food dispensing tongs were stored improperly during periods of nonuse, an ice machine was rusty inside, beverages stored in a reach-in freezer were in need of defrosting, the exterior of a cornmeal/flour container wasn't clean, canned goods and shelving were dusty in retail area, utensils were stored improperly in kitchen, ceiling tiles in kitchen were water damaged and lights weren't shielded on a hot bar.

- Pancho's of East Broadway in Campbellsville received a health code score of 79, citing 11 violations. A refrigerator was not maintaining temperature, chemicals were stored next to chicken, insect killer was stored on a cooler, rust had built up in an ice box, ice scoops were not stored with the handle up, beans were not labeled, seasonings were not labeled, chicken was thawing on the floor, sugar was stored on the floor, rice was stored on the floor, an improper food holding temperature was used, cleaning cloths were not stored properly, single service items were stored on the floor, a deep freeze needed to be defrosted, spatulas were stored between a wall and a sink, there was no shield on a light in a refrigerator, the floors were soiled throughout and the walls and ceiling tiles were soiled throughout.

- Creek Side Restaurant of New Lebanon Road in Campbellsville received a health code score of 91, citing five violations. The back door wasn't protected and does not close properly and its seal was in poor repair, pots and pans were pitted, an ice machine was rusty, an oven had char buildup, paint/floor covering were chipping and ceiling tiles were missing and in poor repair.
