- Long John Silver's and A&W of Campbellsville Bypass in Campbellsville received a health code score of 96, citing four violations. The exterior of a reach-in cooler wasn't clean, a shelving area above a warmer wasn't clean, floor areas under a fryer and in a walk-in cooler weren't clean and the ceiling in the kitchen had soiled tiles and rusty vents.

- Lil T's Traffik Stop of Lebanon Avenue in Campbellsville received a food health code score of 97 and retail health code score of 100, citing three violations. There were no chemical test kits for testing sandwiches, the inside of a donut display case wasn't clean, case boxes of Styrofoam cups were stored on the floor of a storage room and plastic spoons were stored uncovered at a front counter.

No signs were posted instructing patrons to use a clean plate during each trip to the buffet, food products were found uncovered and stored on the floor and under water/condensate lines of a compressor in a walk-in cooler, an ice cream scoop was not stored/rinsed in provided dipping well, a chemical sanitizing dishwasher was not reaching a required 120 F temperature, there was no covered trash can in the women's restroom, tiles near a three-compartment sink were in poor repair, a floor area under storage shelving wasn't clean, there was a hole in the wall of the men's restroom, a wall at a dishwasher was in poor repair and the lights at buffet bars weren't covered.

- Sonic Drive In of Campbellsville Bypass in Campbellsville received a health code score of 96, citing three violations. Non-food counter areas weren't clean, the inside and bottom of two reach-in coolers weren't clean and floors under and behind equipment and shelving in the kitchen area weren't clean.

- Ol' Mule Antique Diner of Hodgenville Road in Campbellsville received a food health code score of 97 and a retail health code score of 99, citing two violations. There was no covered trashcan in the restroom, restroom doors were not self-closing and unshielded lights were found in the kitchen.

A scoop for sugar was stored with the handle in the sugar, shelving under a grill wasn't clean, a walk-in cooler's plastic air drapes weren't clean, there was no covered trash can in the women's restroom, the floor under shelving in a dry storage room wasn't clean, ceiling tiles around a grill area weren't clean, lights over a hot and cold buffet bar were improperly shielded and the temperature of an automatic dishwasher was incorrect.

- Hardee's of East Broadway in Campbellsville received a health code score of 99, citing one violation. Floors weren't clean in hard to reach areas.

- McDonald's of East Broadway in Campbellsville received a health code score of 97, citing three violations. Floors weren't clean in hard to reach areas, lights weren't shielded in a walk-in freezer and a walk-in freezer was not defrosting correctly because of an ice buildup.

- Dawn's Child Care of North Columbia in Campbellsville received a health code score of 100, citing no violations.
