The following businesses were inspected by the Lake Cumberland District Health Department using the scale below.

• 85 to 100, pass inspection.

• 70 to 84, business must be reinspected within 30 days.

• Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

• Rally's of Campbellsville Bypass in Campbellsville received a health code score of 99, citing one violation. A freezer lid was in poor repair.

• Pancho's of East Broadway in Campbellsville received a health code score of 91, citing five violations. Uncovered foods were found on hot bar, a salsa container wasn't covered, foods in a walk-in cooler weren't covered, an ice machine was used as a cooling method for sliced lemons, an ice scoop was stored with its handle down, a dishwasher was found without instruction plate but working properly, a microwave wasn't clean, bulk chip container exteriors weren't clean and a tube of Neosporin was found on a spice rack.

• Happy Days Ole Time Soda Fountain and Grille of East Main Street in Campbellsville received a health code score of 92, citing three violations. No date markings were made on potentially hazardous ready-to-eat food products, raw eggs were stored over produce and there were no paper towels at a hand washing station.

• Center Stage of Lone Valley Road in Campbellsville received a health code score of 94, citing two violations. An ice cream scoop was stored unclean in standing water and hadn't been sanitized within four hours and there were no paper towels at an employee hand sink. The first violation was immediately corrected and the store then received a score of 98.

• Gnomies BBQ of Campbellsville received a health code score of 100, citing no violations.

• Happy Days Ole Time Soda Fountain and Grille of East Main Street in Campbellsville received a health code score of 100, citing no violations.

• Campbellsville High School of West Main Street in Campbellsville received a health code score of 98, citing two violations. The floor under an ice machine/oven wasn't clean and wall piping wasn't clean.

• Kroger of Green River Plaza in Campbellsville received a food health code score of 100 and a retail health code score of 99, citing one violation. Disposable trays were stored on a hand sink basin.

• Little Ceasar's of Broadway in Campbellsville received a health code score of 100, citing no violations.

• Campbellsville Middle School of Roberts Road in Campbellsville received a health code score of 99, citing one violation. The wall/ceiling behind and above a dishwasher wasn't clean and was in poor repair.
