The following businesses were inspected by the Lake Cumberland District Health Department using the scale below.

• 85 to 100, pass inspection.

• 70 to 84, business must be re-inspected within 30 days.

• Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

• Phillips Lanes Restaurant of Bambi Trail in Campbellsville received a health code score of 98, citing two violations. Stockroom shelves were in poor repair and ceiling tiles throughout were in poor repair.

• Sonic Drive In of Campbellsville received a health code score of 93, citing three violations. A can opener was soiled, clean utensils were stored on an unclean surface and an employee restroom didn't have a proper waste receptacle or a self-closing door. In an immediate follow-up inspection, the can opener violation was corrected and the restaurant received a score of 97.

• Burger King of Campbellsville received a health code score of 100, citing no violations.

• Emerald Isle Resort and Marina of County Park Road in Campbellsville received a health code score of 100, citing no violations.

• Baskin Robbins of KY 210 in Campbellsville received a health code score of 100, citing no violations.

• Penn Store & Restaurant of Bradfordsville Road in Campbellsville received a food health code score of 93 and a retail health code score of 100, citing 10 violations. Raw bacon was stored over a ready-to-eat salad, eggs were stored on the top shelf of a refrigerator over ready-to-eat foods, the surface under cups was in poor repair, there were no handwashing signs in the men's restroom, a waste receptacle wasn't properly covered in the women's restroom, hard-to-reach areas on the floor weren't clean, a storage room floor was soiled, the floor of a cooler was soiled and in poor repair, tile flooring through the store was in poor repair and lighting fixtures weren't properly shielded in a food preparation area.

• Circle R of Lone Valley Road in Campbellsville received a health code score of 92, citing six violations. An ice bin under a drink machine wasn't clean and raw wood shelves weren't easily cleaned or sanitized, a three-compartment sink wasn't clean, wiping cloths weren't stored in proper sanitizing buckets, a grill table wasn't clean and the grill hood and counter tops weren't clean, lighting fixtures weren't shielded over a food preparation area and a mop was improperly stored in a bucket.

• Gribbins Grocery of Liberty Road in Campbellsville received a food health code score of 100 and a retail health code score of 96, citing three violations. Shelving under a sink wasn't clean, shelving and the products on the shelves throughout the store weren't clean and vents in the ceiling weren't clean, there were no paper towels at the employee handwashing sink and floors throughout the store were in poor repair.

• Taylor County Elementary School of Lebanon Avenue in Campbellsville received a health code score of 95, citing four violations. Milk crates were stored on the floor and boxes were stored on the floor in freezers, numerous wiping cloths weren't in proper sanitizing solution, the floor under shelving in a stock room wasn't clean and ceiling tiles were in poor repair throughout the kitchen and dining room.

• Taylor County Middle School of East Broadway in Campbellsville received a health code score of 96, citing three violations. Ice machine drip pan was in poor repair, there was no handwashing sign at a kitchen sink and the storage room floor was in poor repair.
