The following businesses were inspected by the Lake Cumberland District Health Department using the scale below.

• 85 to 100, pass inspection.

• 70 to 84, business must be reinspected within 30 days.

• Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

• Mi Nayarit of Nancy Cox Drive in Campbellsville received a health code score of 99, citing one violation. Frozen fish and beef shanks weren't properly labeled for individual resale.

• Garcia's of Campbellsville Bypass in Campbellsville received a health code score of 96, citing three violations. There was no covered waste can in the women's restroom, a floor tie in the kitchen was missing, the floor in a grill area was cardboard lined and the kitchen area and equipment were improperly vented.

• Taylor County Senior Citizens Center of South Jackson Street in Campbellsville received a health code score of 96, citing two violations. A three-compartment sink was used in the improper sequential manner and restroom trashcans didn't have proper lids.

• The Grandview of Water Tower Bypass in Campbellsville received a health code score of 100, citing no violations.

• Sonic Drive-In of Campbellsville Bypass in Campbellsville received a health code score of 94, citing five violations. Boxed food was stored on the floor of a freezer, a shelf was lined with cardboard, a bun rack wasn't clean, boxed cups were stored in an employee restroom and the floor of a walk-in cooler and behind fryers wasn't clean.

• Happy Days Ole Time Soda Fountain and Grille of East Main Street in Campbellsville received a health code score of 95, citing one violation. Employees were using bare hand contact during food preparation of ready-to-eat products. The violation was immediately corrected and the restaurant received a score of 100, citing no violations.

• Green River State Park Grocery of Park Office Road in Campbellsville received a health code score of 99, citing one violation. Snack cakes being sold weren't labeled for individual sale.

• Center Stage of Lone Valley Road in Campbellsville received food and health code scores of 99 and 100, respectively, citing one violation. Wiping cloths were found not in sanitizing solution.

• PeeJay's Green River Lake Quick Stop of Lone Valley Road in Campbellsville received food and health code scores of 97, citing three violations. Shelving wasn't clean, coffee stirrers weren't stored for individual dispensing and the baseboards in the men's and women's restrooms were in poor repair.

• Little Ceasar's of East Broadway in Campbellsville received a health code score of 100, citing no violations.
