• 85 to 100, pass inspection.

• 70 to 84, business must be reinspected within 30 days.

• Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

• Phillips Lanes Restaurant of Bambi Trail in Campbellsville received a health code score of 96, citing four violations. Shelving in a freezer was in poor repair, fountain drink machine nozzles weren't clean, floors throughout the kitchen and by the front counter weren't clean in hard to reach areas and a wall behind a hot water heater and a ceiling fan weren't clean.

• Magic Wok of Campbellsville Bypass in Campbellsville received a health code score of 96, citing three violations. Raw and potentially hazardous foods were stored above ready-to-eat foods, the exterior of a cooler and a knife weren't clean and lights over a buffet bar weren't shielded.
