The following businesses were inspected by the Lake Cumberland District Health Department using the scale below.

• 85 to 100, pass inspection.

• 70 to 84, business must be re-inspected within 30 days.

• Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

• Convenient Food Mart of South Central Avenue in Campbellsville received food and retail health code scores of 95, citing three violations. Raw hamburger packs were stored next to ready-to-eat foods, fountain drink machines weren't clean and restroom sinks were in poor repair and there were no signs at the restroom reminding people to wash their hands.

• Asian Garden Buffet of Central Plaza in Campbellsville received a health code score of 97, citing two violations. There were no signs in the restroom reminding employees to wash their hands and buffet bar lights weren't shielded. The restroom sign violation was immediately corrected.

• Kroger #403 of East Broadway in Campbellsville received food and retail scores of 97 and 100, citing two violations. There were no hand towels at a hand sink and the wall below a fryer and oven wasn't clean.

• Campbellsville Lodge Best Western of East Broadway in Campbellsville received a health code score of 100, citing no violations.

• Ol' Mule Antique Diner of Hodgenville Road in Campbellsville received food and retail scores of 98 and 100, citing one violation. Restroom door wasn't self-closing and a trash can didn't have a lid.

• The Healing Place of Campbellsville of Heistand Farm Road in Campbellsville received a health code score of 97, citing one violation. A can opener was soiled. The violation was immediately corrected and score was changed to a 100.

• Winters Dining Hall of University Drive in Campbellsville received a health code score of 97, citing two violations. Raw shell eggs were stored above ready-to-eat foods in a cooler, salad toppings and dessert cake area had food displayed unprotected and a wall near a dishwasher area wasn't clean.

• Campbellsville Nursing and Rehabilitation Center of Old Greensburg Road in Campbellsville received a health code score of 100, citing no violations.

• Long John Silvers/A&W of Campbellsville Bypass in Campbellsville received a health code score of 99, citing one violation. Floors weren't clean in hard to reach areas.

• Arby's of Lakeway Shopping Center in Campbellsville received a health code score of 100, citing no violations.

• Wise Beginning of West Main Street in Campbellsville received a health code score of 100, citing no violations.

• Druther's of North Columbia Avenue in Campbellsville received a health code score of 88, citing four violations. Employees were using bare hand contact with ready-to-eat foods, an employee's drink was on a food preparation area, the wrong type of sanitizer test strips were being used, employees weren't going through the proper wash cycle while washing dishes and the floor under a fryer wasn't clean. The bare contact, employee drink and washing cycle violations were immediately corrected and the restaurant received a health code score of 97.

• Gatti's Pizza of East Broadway in Campbellsville received a health code score of 98, citing two violations. A storage shelf in the food preparation area wasn't clean and plates weren't stored inverted.

• Lil T's Traffic Stop of Lebanon Road in Campbellsville received food and retail health code scores of 100, citing no violations.
