• 85 to 100, pass inspection.

• 70 to 84, business must be reinspected within 30 days.

• Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

• Fastrack #8 of New Columbia Road in Campbellsville received a health code score of 98, citing two violations. Potentially hazardous foods were stored out of temperature, though the violation was corrected and the food was thrown away, and the restroom door wasn't self-closing.

• Hardee's of East Broadway in Campbellsville received a health code score of 98, citing two violations. Eggs weren't at correct temperature, though the violation was corrected and the food was thrown away, and a mop was stored in dirty mop water.

• All Star Food Mart of Elk Horn Road in Campbellsville received a food health code score of 98 and a retail health code score of 100, citing one violation. A restroom door wasn't self-closing.

• Jackson Gas and Wash Inc. of Jackson and Broadway in Campbellsville received retail and health code scores of 97, citing two violations. There were no signs at either restroom or kitchen sink reminding employees to wash their hands and floor tiles in both restrooms were in poor repair.

• Papa John's Pizza of Central Plaza in Campbellsville received a health code score of 100, citing no violations.

• Lee's Famous Recipe of East Broadway in Campbellsville received a health code score of 82, citing nine violations. Raw eggs were stored above produce, employee drinks were found on food preparation surface, cups were used as scoops, a freezer door was rusty, dishes weren't being washed properly, equipment wasn't sanitized within four hours of use, wet wiping cloths weren't stored in sanitizing solution, food storage containers were soiled, there were no paper towels at a hand sink, floors were soiled in the food preparation area and in poor repair in some places and the walls were soiled in the food preparation area. Several of the violations were immediately corrected and the score was changed to 91.

• Creek Side Restaurant of New Lebanon Road in Campbellsville received a health code score of 92, citing five violations. There was no consumer advisory found in the menu, potentially hazardous food were stored next to ready-to-eat foods, lemon slices weren't covered, salad topping weren't stored under a sneeze guard, a sprayer at the dishwashing area wasn't working, a floor was in need of repair and ceiling tiles were water damaged.

• Dollar General Store #7366 of East Broadway in Campbellsville received a health code score of 100 citing no violations.

• Domino's Pizza of East Broadway in Campbellsville received a health code score of 100 citing no violations.

• Fivestar Food of East Broadway in Campbellsville received a health code score of 96, citing one violation. A rubber hose connected to a mop sink didn't have a backflow preventer. The violation was immediately corrected and the score was changed to 100.

At an inspection two weeks later, the restaurant received a health code score of 100, citing no violations.

• Save-a-Lot of Lakeway Shopping Center in Campbellsville received a health code score of 95, citing two violations. A mop sink was connected without a back flow preventer and lights weren't shielded in some walk-in and reach-in coolers. The backflow violation was corrected and the store received a score of 99.
