The following businesses were inspected by the Lake Cumberland District Health Department using the scale below.

  •  85 to 100, pass inspection.
  •  70 to 84, business must be reinspected within 30 days.
  •  Less than 70, a suspension of permit notice is sent to owner; if owner doesn't reply within 10 days, the permit will be revoked and the business will be closed.

 

  •  Mini Mart of Campbellsville Bypass in Campbellsville received a retail health code score of 98 and food score of 97. Bagged ice for retail sale was improperly labeled and restroom doors weren't self-closing.
  •  Dollar General Store #7366 of East Broadway in Campbellsville received a health code score of 99. Single service items - towels, cups and plates - were stored in a case on the floor of a stockroom.
  •  Taco Bell of East Broadway in Campbellsville received a health code score of 100, citing no violations.
  •  Domino's Pizza of East Broadway in Campbellsville receive a health code score of 100, citing no violations.
  •  Rally's of Campbellsville of Campbellsville Bypass in Campbellsville received a health code score of 95. Thermometers weren't found in coolers or freezers throughout, deep fryer at a food preparation area was in poor repair, a hot hold cabinet, toppings area and soda foundation were soiled and a mop sink faucet was in poor repair.
  •  Dairy Queen of West Main Street in Campbellsville received a health code score of 100, citing no violations.
  •  L. Teg Food Stores of East Broadway in Campbellsville received a health code score of 96. Single service items were stored on the floor and next to a trashcan, the sink in women's restroom was in poor repair, a mop sink was inaccessible, restroom doors weren't self-closing and there was no lid on trashcan in women's restroom.
  •  Five Star Food Mart of East Broadway in Campbellsville received a retail health code score of 99 and a food score of 99. Cabinets and shelving were soiled.
  •  Jackson Gas & Wash Inc. of Jackson and Broadway in Campbellsville received retail and food health scores of 100, citing no violations.
  •  Allstar Food Mart #22 of Elk Horn Road in Campbellsville received a retail health code score of 100 and a food score of 98. Sandwich topping cooler wasn't clean, an ice machine had buildup and lids for single service items were stored on the floor.
  •  Phillips Lanes Restaurant of Bambi Trail/Greensburg Road in Campbellsville received a health code score of 99, citing one violation. Ceiling tiles were in poor repair.
  •  Puff 'N Chew of South Central Avenue in Campbellsville received a health code score of 100, citing no violations.
  •  McDonald's of East Broadway in Campbellsville received a health code score of 83. Hamburger patties and cookies were stored unprotected in a reach-in freezer, employee drinks were found in drink preparation area, wiping cloths throughout weren't stored in sanitizing solution, coffee area, cooler doors, bottom of cooler, soda foundation in dining area and cabinet under soda foundation were soiled, ice machine had buildup in freezer, single service cups, coffee and filters and paper towels were stored on the floor, single service items were soiled because of water from leaking sink, mop sink plumbing was in poor repair, floors were soiled in hard to reach areas, floor was in poor repair in a storage area, ceiling was in poor repair in storage and dining area and lights weren't shielded at drink area and drive-thru. At a follow up inspection, the restaurant received a health code score of 99, citing one violation. Ceiling tiles were in poor repair in storage area.
  •  Scotty's BBQ of Speck Ridge Road in Campbellsville but located at Save-A-Lot, received a health code score of 98, citing one violation. Meat wasn't protected in a cooler.
  •  Pancho's of East Broadway in Campbellsville received a health code score of 88. Potentially hazardous food wasn't stored at 45 degrees or cooler, thermometers weren't found in refrigerators and freezers, pans weren't properly transported at food preparation area, seasonings weren't covered, a can opener and microwave were soiled, floors were soiled throughout and a vent over a preparation table was in poor repair. At a follow-up inspection, the restaurant received a health code score of 100, citing no violations and having corrected all previous violations.
  •  Snappy Tomato of West Main Street received a health code score of 100, citing no violations.
  •  Druther's of North Columbia Avenue in Campbellsville received a health code score of 92. Thermometers weren't found in a conspicuous place in reach-in deep freezer, products were stored on the floor of a walk-in cooler, eggs were stored next to ready-to-eat products, cooler door was in need of repair, a microwave was soiled, buildup was found in an ice machine, floors were soiled in hard-to-reach areas and in a walk-in cooler, walls near a three-compartment sink were in poor repair and shelving/walls were soiled.
  •  Gribbin's Grocery of Liberty Road in Campbellsville received a food health code score of 97 and a retail code score of 99. Thermometers weren't conspicuous in refrigerator, freezer and chest deep freezer and shelving was dusty throughout.
  •  Café Bonin of East Main Street in Campbellsville received a health code score of 97. No conspicuous thermometers were found in a reach-in cooler, freezer and toppings cooler and the floor of a walk-in cooler was soiled.
  •  Asian Garden of Central Plaza in Campbellsville received a health code score of 86. Chicken was thawing at unapproved room temperature, raw meats were stored over ready-to-eat foods, product was stored unprotected in coolers, scoops of flour at food preparation area weren't approved and stored with handle in product, employee drinks were found in food preparation area, hand sink, can opener, bottled product, ice machine and door, coolers and doors were soiled, floor in storage area wasn't smooth or easily cleanable, floors were soiled in walk-in freezer and in hard-to-reach areas and lights weren't shielded at buffet. At a follow-up inspection, the restaurant received a score of 96. Raw meats were stored over ready-to-eat foods, products were stored unprotected in walk-in cooler and freezer, a hand sink was soiled and floors were soiled in walk-in freezer and in hard-to-reach areas.
  •  Emerald Isle Marina of County Park Road in Campbellsville received a food health code score of 96 and a retail score of 100. No thermometers were found in a pizza topping cooler and there was no lid on a trashcan in the women's restroom.

  •  Garcia's of Campbellsville Bypass in Campbellsville received a health code score of 97. Non-food grade pastry brushes were found and there was no sign at kitchen hand sink reminding employees to wash their hands and no lid on the trashcan in the women's restroom.
  •  Baskin Robbins of KY 210 in Campbellsville received a health code score of 100, citing no violations.
