• Best Western Campbellsville Lodge in Campbellsville received a health code score of 100, citing no violations.

• Circle "R" at the Lake of Lone Valley Road in Campbellsville received a health code score of 100, citing no violations. At a later inspection, the restaurant received a food health code score of 97 and a retail health code score of 100, citing two violations. Thermometers weren't found in freezers and shelving wasn't smooth or easily cleanable.

• Little Ceasars of East Broadway in Campbellsville received a health code score of 95, citing three violations. Thermometers weren't conspicuous in a toppings cooler, scoops weren't stored with the handle up and a can opener and paper towel holder were heavily soiled.

• Emerald Isle Resort of County Park Road in Campbellsville received retail and health code scores of 100, citing no violations.

• Mi Nayarit of Nancy Cox Drive in Campbellsville received a health code score of 99, citing one violation. Fish wasn't properly labeled.

Cheese, butter, sour cream and egg wash were out of temperature, dishwasher rinse cycle wasn't reaching 180 degrees and flies were found throughout the restaurant. At a follow-up inspection, the restaurant received a health code score of 98, citing one violation. A gauge on the dishwasher was in poor repair.

• Dollar General Store #13882 of New Columbia Road in Campbellsville received a health code score of 92, citing three violations. Several severely dented cans were found, freezers weren't maintaining the proper 0 degree temperature and thermometers weren't conspicuous throughout the store. At a follow-up inspection, the store received a health code score of 98, citing one violation. Thermometers weren't conspicuous throughout.
